Sample Codicil
When it’s time to update your will, you don’t have to start from scratch.
You can use a codicil, an addition or supplement that explains or modifies your existing will.
I/we give, devise and bequeath the sum of [$ ________] (or percentage of ____% of my estate), to Mass General Brigham, Inc., (“MGB”), an integrated health system and a Massachusetts not-for-profit, 501(c)(3) tax-exempt corporation having a principal place of business at 399 Revolution Drive, Somerville, MA 02145 (c/o Development Office) federal tax identification number of 04-3230035, to be used for its general purposes.
Should it become impossible or impracticable to carry out the designated purpose, I authorize the trustees of Mass General Brigham, Inc. to determine an alternate purpose as near as possible to my wishes as stated herein.
In all other respects I ratify and confirm all of the provision of my said last Will and Testament dated on the _________ day of ______________, 20__.
IN WITNESS WHEREOF, I sign, publish and declare this instrument to be a Codicil to my last Will and Testament in the presence of the persons witnessing said Codicil at my request this _____ day of ___________, 20__.

The codicil, to be valid, must be signed in the presence of witnesses, and the original must be placed with your other valuable papers. A copy can go to your lawyer, and another to Mass General Brigham.
